Before your first appointment, download our 'New Patient' form using this button. Either complete the form on your computer or by hand, and make sure to bring it with you to your appointment.
If you wish to transfer out from your current medical centre to Wyee Medical Centre, you will first need to complete this 'Transfer of Patient Records' form.
Sometimes we will need to acquire
information from other medical professionals that you have attended. When this is required, we will ask
you to complete the above form.
The Urgent Care Clinic is available
for individuals who require medical care that is not immediately life threatening. The above button provides a resource with more information about the clinic.
